For your convenience you can download the MSWord version of this form.  Feel free to adapt it to fit your classroom needs.  Place the form in the Course Content area of your classroom.  The best way to collect the forms back from your students it to create a gradebook item so that the student can submit the form just as they would any other assignment. 

The peer evaluation rating form is designed to allow each group member to asses the level of effort of each put forth by the other members of the team FOR ALL GROUP ASSIGNMENTS, and, in particular, the group case paper. A copy of this form will be placed in the Course Content area.

Instructions

Each member of your group must download an evaluation form from the study group area, complete it, and submit it to his or her individual assignment folder (NOT in the study group area!) as an attachment no later than the due date and time noted on the course schedule (found in the Syllabus area of the online classroom).

Students should carefully review the criteria listed on the form to determine if each group member's participation in the group projects meets expectations. Answer the questions in the form for each team member and resave the file.

Be sure to note your own name and the group members' names in the first row.

Rating Scale: 1-5

For each item, rate each group member, excluding yourself, on the following points using a scale of 1-5 where

1 = very poor; did very little; not a team player
2 = poor; did only what was minimally required; not a positive member of the team
3 = about right; met reasonable expectations; a member of the team
4 = good; performance was above average for the team
5 = excellent; effort went above and beyond the call of duty; a team leader.

How The Instructor Will Calculate Grades

The total score for each team member will be multiplied by 2 and divided by 10 to equal 10% of the course grade.

For example, if the total score is 40, then: 40 x 2 = 80, 80/10 = 8, 8 of 10 = B.

The instructor will make these calculations based on an average of the ratings provided for each team member by each other member of the team and will not supplant his judgment for the evaluation other group members have made of one another.

The instructor will keep each student's evaluation of the other group members strictly confidential.
